We can accept faxed registration forms until 2 p.m. Saturday, March 11. After that, you are welcome to register on site. Symposium Registration will be open on the Independence Level of the Grand Hyatt during the following hours:

Sunday, March 12 3:00 p.m. 7:00 p.m.
Monday, March 13 7:00 a.m. 5:00 p.m.
Tuesday, March 14 7:00 a.m. 5:00 p.m.
Wednesday, March 15 8:00 a.m. 3:30 p.m.

Cancellation/Substitution Policy Cancellation requests must be in writing and received (NOT POSTMARKED) no later than March 1, 2006. Registration fees minus a $75 handling fee will be refunded. Refunds will be made in the same form of payment as originally received. If the cancellation is not received by March 1, 2006, the registration fee will be forfeited. Registration may be transferred to a member of your agency/institution and must be received in writing via email to Laura DeOrio, Events Manager, at prior to March 6, 2006.

Tax Deductible An income tax deduction may be allowed for educational expenses. Consult your tax advisor.
